For your convenience, most of our supplemental and advisory rate applications
are available below as PDF files. In order to view these applications you will
need Adobe Acrobat Reader installed on your computer. This is a free
application and can be downloaded by clicking
here.
All supplemental applications must be submitted with the appropriate Acord
application. Some of the applications are in an editable PDF format. Click on
the first field and type in your response. You can move to the next field by
using the tab key (use the shift and tab keys to move back) or with your mouse.
You will also find check boxes on some forms. These will allow you to select
your response by clicking on the appropriate box or by pressing the space bar.
Once the form is complete, you can print it by clicking on the print button on
the bottom of the last page. The clear button will clear the entire form and
allow you to start over. The submit button will electronically submit the data
on the form. Please make sure the form is complete and correct prior to
submitting. Applications submitted electronically will be quoted and returned
by fax. Advisory raters will be bound effective date and time received by RIC,
or a later requested date.
